Insurance & Billing

• Charges at Hayes Valley Surgery Center consist of a global fee for each procedure performed. Charges for anesthesia are billed separately. An estimate will be provided to you prior to surgery, but additional charges may be necessary if the time used exceeds the estimated time scheduled.

• Prior to your surgery date, you will be contacted regarding your insurance coverage. Due to the number of insurance plans, it is your responsibility to contact your insurance company if you have any questions regarding insurance coverage.

• After your surgery, we will file the insurance papers for you and ask you to sign an assignment of benefits so the payment will be sent directly to the surgery center. Any amounts not covered by your insurance (i.e. deductibles and coinsurances) will be billed to you.

• Additionally, a member of our Scheduling/Billing team will contact you if any portion of your procedure is covered by insurance.

• If you are self-paying, your surgeon’s office will often provide pricing information for our center - however, you are always welcome to contact us directly with any inquiries.
